Parent and Guardian Consent Form

Effective 5 November 2026

Please complete this form when your child starts at KieAn. Online bookings already include consent to use your booking details; this form adds the information we need for your child’s safety. Print it, or fill it in on screen and print or save it as a PDF, then sign and hand it to your branch.

Please bring a valid ID when picking up. We release children only to the parent or guardian or to the persons listed here.

F. Consents

1. I consent to KieAn collecting and using my and my child’s personal information as described in the Privacy Policy. (Required to enroll.)
2. In an emergency, I authorize KieAn staff to seek first aid or medical help for my child if I cannot be reached right away.
3. KieAn may use photos or videos of my child taken during activities on its website and Facebook Page. (Optional; this does not affect enrollment.)
4. For online lessons, I will make sure an adult is nearby and that lessons are not recorded without the teacher’s consent.
5. I have read the Terms and Conditions and the Refund Policy.

You may withdraw any optional consent at any time by messaging KieAn on Facebook.
